eForm Request
Things you need to do before you submit an eForm request:
- Please check to make sure the eForm has not already been requested (pending), in progress, or completed. You can search for the name of the form using the search box in the upper right corner of this page.
- If the eForm has already been created, follow these instructions to download the eForm to your OSCAR system. You must have the appropriate ADMIN privilege to do this.
- If the eForm has not been requested before please print this page, fill in the requested information, and submit it to the appropriate person in your unit for authorization. Remember to include your communication address (email or oscar username) so we can keep you informed of the progress. You will need one request form per eForm request.
- You will need a clean copy of the form for scanning.
- You will need another copy of the form indicating fields on the form for data input.
- Remember that there is a cost for producing an eForm - from $15 for the simplest type to $40 for a complex form with many input points. If the cost is more than $40 we will notify you ahead of time.
